It is very important.  Positive and Negative offsets
move your wheels in different directions.  When you
see the lowered trucks with the wheels sticking way
outside the fenders, those wheels have negative
offsets.  A positive offset moves your wheel in the
other direction, towards the inside of the wheel well.
 So depending on your offset + or - you move your
wheel in a different direction.
